The Nurse Navigator coordinates the delivery of patient care by assessing care requirements of the individual patient and makes recommendations based on severity of illness, intensity of service and skill level. The Nurse Navigator ensures patients receive timely, safe, effective and high quality care. They coordinate the patient's plan of care with the bedside nurse. The Nurse Navigator sets the expectations regarding professional conduct and serves as a resource for the patient care unit. The Nurse Navigator will hold direct and indirect care providers accountable for their professional responsibilities, and adherence to regulatory guidelines. The Nurse Navigator is responsible for management for the assigned inpatient, and outpatient population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety, and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Responsibilities include; pre-op education, daily rounding while the patient is in the hospital, patient discharge disposition, and follow-up care. We're seeking an experienced Registered Nurse to join our team as a Nurse Navigator. In this role, you'll serve as a trusted guide for patients throughout their healthcare journey, helping coordinate care, improve outcomes, and ensure a seamless experience from pre-treatment through follow-up care.

The Nurse Navigator partners closely with patients, families, physicians, bedside nurses, and interdisciplinary teams to assess needs, coordinate care plans, and connect patients with the resources and support they need. This position plays a key role in patient education, care coordination, discharge planning, utilization management, and quality improvement initiatives.

What You'll Do
  • Coordinate care for assigned inpatient and outpatient populations
  • Guide patients through their treatment journey, serving as a primary resource and advocate
  • Provide patient and family education, including pre-operative preparation and follow-up care
  • Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care
  • Support discharge planning and coordinate post-acute services
  • Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care
  • Assist with length of stay (LOS) management and utilization review activities
  • Participate in quality improvement initiatives and implementation of best practices
  • Serve as a clinical resource and mentor to members of the care team
  • Maintain accurate documentation within the electronic medical record (EPIC)
Qualifications

Required

  • Graduate of an accredited School of Nursing
  • Current RN license in the state of practice
  • Minimum of 2-3 years of nursing experience
  • BLS certification

Preferred

  • Bachelor of Science in Nursing (BSN)
  • ACLS certification
  • Experience in care coordination, case management, utilization management, or patient navigation

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 185 years. It began with an Irish woman named Catherine McAuley, who wanted to help the poor women and children of Dublin. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill her dreams. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to poor women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, the first religious order not bound to the rules of the cloister, whose Sisters were free to walk among the poor and visit them in their homes.
