Remote Physical Therapist | Hands of Hope Physical Therapy | Handshake

We are looking for a committed Physical Therapist to support patients remotely through virtual consultations, individualized care planning, education, progress monitoring, and evidence-based rehabilitation guidance.

The Physical Therapist will work closely with patients to help improve mobility, strength, flexibility, balance, and overall function while providing professional and personalized support through remote communication and telehealth systems.

Responsibilities
  • Conduct remote patient assessments and virtual consultations.
  • Identify patients' mobility, rehabilitation, and functional goals.
  • Develop individualized treatment and rehabilitation plans based on patient needs.
  • Provide patients with appropriate therapeutic exercise and mobility programs that can be performed safely at home.
  • Educate patients on movement, posture, injury prevention, wellness, and rehabilitation techniques.
  • Monitor patient progress through scheduled virtual follow-ups.
  • Adjust care plans and exercise programs based on patient progress and feedback.
  • Motivate and support patients throughout the rehabilitation process.
  • Promote healthy lifestyles by helping patients improve strength, flexibility, balance, coordination, and mobility.
  • Review patient information and maintain accurate clinical documentation.
  • Communicate patient progress and relevant concerns with other healthcare professionals when appropriate.
  • Provide personalized, evidence-based care and rehabilitation guidance.
  • Maintain professional communication with patients through approved remote communication and telehealth platforms.
  • Follow all applicable patient privacy, documentation, and professional practice requirements.
